Quick answer: No study has shown that creatine causes hair loss. The worry traces back to one small 2009 trial in which 20 male rugby players took a 25 g/day loading dose and showed a rise in DHT, a hormone linked to pattern hair loss. Nobody's hair was measured, and no trial since has found more shedding on creatine. For women, hair loss after starting creatine is far more likely to have another cause: postpartum shedding, low iron, thyroid changes, or a crash diet that began the same month. If you have PCOS or pattern hair loss in the family, skip loading and stay at 3 g a day rather than giving creatine up.

Where the creatine hair-loss rumor started
The link comes from a single paper: van der Merwe, Brooks and Myburgh, Clinical Journal of Sport Medicine, 2009.
- Who: 20 college-aged male rugby players.
- What: 25 g of creatine a day for 7 days, then 5 g a day for 14 days, compared with a placebo period.
- Measured: Blood testosterone and dihydrotestosterone (DHT).
- Found: Testosterone didn't change. DHT rose during the loading week and stayed above baseline through maintenance, while remaining within the normal reference range.
- Not measured: Hair. No hair counts, no scalp photos, no questions about shedding.
One study, one sex, an unusually high loading dose, and a hormone reading never connected to anyone's hairline. The DHT finding hasn't been reproduced in later trials that looked for it.
DHT and hair: why the worry sounds plausible
DHT does matter for hair. In androgenetic alopecia, the hereditary type of hair loss, DHT acts on follicles that are genetically sensitive to it, gradually shrinking them so hairs grow finer and shorter. The rumor's logic: creatine raises DHT, DHT shrinks follicles, so creatine causes hair loss.
The chain breaks twice. The DHT rise stayed inside the normal range and hasn't been confirmed elsewhere. And DHT only thins hair in follicles primed for it: someone without that sensitivity can have plenty of DHT and a full head of hair, while someone with it usually thins over years regardless of supplements. Creatine has never been shown to flip that switch.
What the reviews concluded
The 2021 review by Antonio and colleagues in the Journal of the International Society of Sports Nutrition addressed the question directly. Of twelve studies that measured testosterone or related hormones in people taking creatine, the rugby study was the only one reporting a DHT increase, and none measured hair loss. Their conclusion: current evidence does not support the claim that creatine causes hair loss or baldness. The 2017 ISSN position stand, which summarises decades of safety data, doesn't list hair loss among documented side effects either.
Does creatine cause hair loss in women?
Even less likely than in men. Women have much lower androgen levels to begin with, female pattern hair loss is usually diffuse over the crown rather than a receding line, and DHT's role in it is less clear-cut. Whatever a 25 g loading dose did to hormone readings in male rugby players was never tested in women, and a woman on 3–5 g a day is nowhere near that protocol.
The bigger point: most women who notice shedding after starting creatine started something else at the same time. Creatine is the new tub in the cupboard, so it gets the blame.
If you have PCOS or hair loss in your family
This is the one group where a little caution is reasonable, not because creatine is proven to cause a problem but because you have less margin. Women with polycystic ovary syndrome often already run higher androgens, and hereditary pattern loss means your follicles are the sensitive kind.
- Skip the loading phase. The only study that showed a DHT change used 25 g a day. Loading isn't necessary anyway; a daily dose fills muscle stores in three to four weeks. See Do you need to load creatine?.

- Tell whoever manages your PCOS or hair. Not because of a known interaction, but so any change in shedding is judged with the full picture.
- Take baseline photos. Same parting, same light, once a month. Hair changes slowly and memory is unreliable.

Shedding that gets blamed on creatine
Hair loss in women is common, usually lags its cause by two to three months, and tends to arrive around big life changes, which is exactly when people also start a new supplement. Rule these out first:
- Postpartum shedding. A sudden shed three to four months after birth is normal and self-limiting. Starting creatine when you return to training lines up with it almost perfectly.
- Low iron. Heavy periods, vegetarian diets and hard training push iron down, and low ferritin is one of the most common contributors to diffuse thinning in women. A blood test settles it.
- Thyroid changes. Both under- and overactive thyroid shed hair. Fatigue, temperature sensitivity and cycle changes alongside hair loss deserve a check-up.
- Crash diets and rapid weight loss. A sharp calorie cut or a low-protein phase commonly triggers shedding two to three months later. "Creatine for weight loss" routines usually involve the diet, not the creatine.
- Stress, illness, surgery. A big physical or emotional shock can push a wave of hairs into the shedding phase at once (telogen effluvium). It grows back once the trigger passes.
The American Academy of Dermatology lists all of these among everyday causes of hair loss; a dermatologist can usually tell them apart from the pattern and a few questions about the last few months.
How to test whether it's the creatine
Hair works on a slow cycle, so short breaks tell you nothing.
- Stop creatine for 8 weeks. Change nothing else: keep training, diet, sleep and hair routine the same. Muscle stores drain over about four weeks, so eight weeks is a real off period.
- Count and photograph. Note how much hair is in the brush or drain each week; take the same parting photo at the start and end.
- Judge the trend, not one bad week. Shedding varies with season, cycle and stress.
- Restart at 3 g a day without loading. If shedding didn't change while you were off, creatine wasn't the cause. If it improved, restart low and watch for another eight weeks before concluding; coincidence is common with hair.

Can stopping creatine reverse hair loss?
If creatine had nothing to do with the shedding, stopping it won't help, and the real cause carries on. If a rare person genuinely reacts to it, hormone-driven thinning is partly reversible in its early stages once the trigger is removed, but it takes a full hair cycle of several months. Hereditary pattern loss progresses on its own timetable either way.
What the real risks of creatine are
For healthy adults at 3–5 g a day, the documented side effects are minor: roughly 1–2 kg of temporary water weight inside muscle in the first weeks (see Weight gain and bloating), and stomach upset from large single doses during loading. On kidneys, the position stand found no harm to kidney function in healthy people at normal doses, though anyone with kidney disease, or who is pregnant or breastfeeding, should ask a clinician first. After decades of trials, the only hair-related evidence against creatine is a hormone reading in 20 male rugby players on a heavy loading dose, with hair never measured and the finding never repeated. That isn't proof it can't affect anyone's hair; it's a reason to keep the worry in proportion. If your hair is thinning, look at iron, thyroid, diet, stress and timing first. If you're in a higher-risk group, skip loading and stay at 3 g. If you want certainty, an eight-week break will tell you more than any forum thread.

Can creatine affect my hair?
There's no evidence that it does. One 2009 study found a DHT rise in men on a 25 g loading dose but didn't measure hair; a 2021 review of twelve studies found no support for the claim.
Does the creatine loading phase cause hair loss?
Loading is the only protocol ever tied to a DHT change, and even there hair wasn't measured. If it concerns you, skip loading; 3–5 g a day reaches the same stores in three to four weeks.
Can stopping creatine reverse hair loss?
Only if creatine was the cause, which is rarely the case. Shedding from iron, thyroid, postpartum timing or dieting continues regardless. Any recovery takes months, so judge over at least eight weeks.
Is creatine good for skin and hair?
Not in any proven way. Creatine appears in some cosmetics, but there's no solid evidence that taking it improves hair or skin. Its benefits are for muscle, strength, recovery and possibly brain function.
